Welding Area - On or about May 2, 2013, the employer did not conduct daily inspections of the 3 ton overhead crane system to move pieces of metal ( R & M Materials - Serial # 1750929501). The operating mechanism for the crane's hook had its safety latch missing. Employees were thereby exposed to struck by hazards.

Lunch Area/Storage Area - On or about May 2, 2013, the employer had several oxygen cylinders and were not separated from acetylene and propane by a minimum distance of at least 20 feet. Employees were thereby exposed to struck by hazards.

Employee lunch/break area - On or about May 8, 2013, the employer did not establish a housekeeping schedule to reduce chromium (VI) in all forms to the lowest level practicable. 1. The top surface (wipe sample # 3) of the table in the lunch area contained 0.0992 micrograms per 10 square centimeters of area of chromium (VI) and 0.1908 per 10 square centimeters of area of chromic acid and chromates.
