29 CFR 1926.760(a)(1): Each employee engaged in a steel erection activity who is on a walking/working surface with an unprotected side or edge more than 15 feet (4.6 m) above a lower level was not protected from fall hazards by guardrail systems, safety net systems, personal fall arrest systems, positioning device systems, or fall restraint systems:     The employer does not ensure that employees are protected from falling while engaged in steel erection activity at heights greater than fifteen feet. This violation occurred on or about April 30, 2013, when an employee was exposed to a fall hazard while installing metal sheeting trim on a roof that was more than 15 feet tall without any means of fall protection.       Pursuant to 29 C.F.R. 1903.19, within ten (10) calendar days of the abatement date, the employer must submit documentation showing that it is in compliance with the standard, including describing the steps that it is taking to ensure that all employees performing steel erection activities are protected from falling.

29 CFR 1926.1053(b)(1): Where (a) portable ladder(s) was/were used for access to an upper level landing surface and the ladders length did not allow the ladder side rails to extend at least 3 feet (0.9 m) above the upper landing surface being accessed, the ladder was not secured at its top to a rigid support that will not deflect, and/or a grasping device was not provided to assist employees in mounting and dismounting the ladder.     The employer does not ensure that all portable ladders used by employees to access an upper landing have their side rails extended at least three feet above the landing or that it is secured at its top to a rigid support to prevent employees from falling. This violation occurred on or about April 30, 2013, when an employee was exposed to a fall hazard while using an extension ladder to access a roof without extending the ladders side rails at least three feet above the roof landing or securing the top of the ladder to the roof landing.      Pursuant to 29 C.F.R. 1903.19, within ten (10) calendar days of the abatement date, the employer must submit documentation showing that it is in compliance with the standard, including describing the steps that it is taking to ensure that all ladders are set up and used in accordance to the requirements of 29 CFR 1926.1053.
