29 CFR 1926.451(b)(1): Each platform on all working levels of scaffolds was not fully planked or decked between the front uprights and the guardrail supports: On or about April 30, 2013, an employee was observed walking and working on a fabricated scaffold platform that was not fully planked when performing brick restoration at the height of 5 feet or more above the ground, exposing the employee to fall hazards.
