29 CFR 1926.100(a): Employees working in areas where there was a possible danger of head injury from impact, or falling or flying objects, or from electrical shock and burns, were not protected by protective helmets:  On or about May 1, 2013, an employee performing demolition activities removing siding and insulation from the exterior side of the building and working near and under workers performing construction activities was not wearing a hard hat and exposed to head injuries from falling or flying objects.

29 CFR 1926.451(c)(2)(iv): Front-end loaders and similar pieces of equipment were used to support scaffold platforms when they were not specifically designed by the manufacturer for such use:   On or about May 1, 2013, an employee performing demolition removal of siding and insulation from the side of a building was working from an unsecured pallet platform lifted in the air by a SkyTrak 5028 rough terrain tele-handler forklift.  The employee was exposed to the hazard of falling from a height up to 16 feet to the ground below.
