29 CFR 1926.501(b)(10): Each employee engaged in roofing activities on low-slope roofs with unprotected sides and edges 6 feet or more above lower levels, was not protected from falling by guardrail systems, safety net systems, personal fall arrest systems, or a combination of warning line system and guardrail system, warning line system and safety net system, or warning line system and personal fall arrest system, or warning line system and safety monitoring system.  Or, on roofs 50-feet (15.25 m) or less in width, each employee was not protected by use of a monitoring system.     a. Metal Roofing Project on the Men's Shelter:  An employee was not protected from a fall of approximately 20 feet from the roof eaves to the ground while working from the peak of the 4 on 12 sloped roof during the installation of the metal roof as observed on 5-1-13. Metal Roofing Project at the Men's Shelter: An employee was not protected from a fall of an estimated 6 to 10 feet to a lower roof while working at the peak of a hip roof that was sloped at 5 on 12 as observed on 5-1-13.

29 CFR 1926.1053(b)(1): Portable ladders were used for access to an upper landing surface and the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface to which the ladder was used to gain access:     a. Single Story Addition to the South End of the Men's Shelter: The 6 foot Werner fiberglass step ladder located on a lower roof and used to gain access by the employees to the roof of the main structure extended above the landing less than one foot as observed on 5-1-13.    b. West End of Single Story Addition to the South End of the Men's Shelter:  The Werner fiberglass extension ladder used to gain access to the roof area only extended above the landing 28 inches on the lower side of the sloped roof in lieu of 3 feet as observed on 5-1-13.

29 CFR 1926.1053(b)(4): Ladder(s) were used for purposes other than the purpose for which they were designed:   a. Single Story Addition Toward the South End of the Men's Shelter: The employees were using a 6 foot fiberglass Werner step ladder that was set up on a lower roof and leaned against the gutter of the upper roof eave to gain access to the roof of the main structure in lieu of having the legs of the ladder spread out and locked in place as observed on 5-13-13.
