29 CFR 1926.20(b)(2): The employer did not initiate and maintain programs which provided for frequent and regular inspections of the job site, materials and equipment to be made by a competent person(s):  (a) Employees were working from scaffold with missing guardrails and had not been instructed to protect themselves by installing guardrails.  (b) Employees were working from open sided decks and had not been instructed to install missing guardrails.  (c) Employees were using full body harness in a manner that would not provide fall protection and workers had not been instructed on the safe use of fall protection.  (d) An employee was stocking sheathing onto a roof under construction while standing in a box on an articulating boom vehicle and had not been instructed on proper use of fall protection while working from an elevated platform.

(b) Employees were working from open sided decks and had not been instructed to install missing guardrails. 29 CFR 1926.1052(a)(1): Stairways that would not be a permanent part of the structure on which construction work was being performed did not have landings of not less that 30 inches (76 cm) in the direction of travel and extend at least 22 inches (56 cm):  (a) The steps used to enter and leave the general contractor's job trailer did not have a landing at the top of the stairs just outside of the exit door.
