29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks: The employer failed to properly guard equipment to prevent injuries such as amputations, fractures, dislocation of bones and lacerations when operating the equipment listed below. 1. Threaders - Machine 507-1, 508-1, 509-1, 503-1, 505-1 guards were not secured. Rotating components were accessed for minor adjustments. 2. Tool Room - 75 Ton KRW Wilson Hydraulic Press, Serial 2125. Machine guarding permits access to the point of operation. - 200 Ton Behr Hydraulic Press. Machine guarding permits access to the point of operation. 3. Headers - Machines 110-1, 115-1, 120-1, 120-2, 105-1, 210-1, 220-1, 230-1, 125-1 guards were not secured and permit access to rotating components. Abatement documentation is required for this item in accordance with the requirements of 29 CFR 1903.19(d).

29 CFR 1910.1200(f)(5): The employer did not ensure that each container of hazardous chemicals in the workplace was labeled, tagged, or marked with the identity and appropriate hazard warnings regarding the chemicals contained therein: The employer did not ensure that all chemical containers and chemicals transferred into unmarked containers were properly labeled. Employees were exposed to chemicals such as but not limited to basic industrial products coolant, degreaser and oil. Abatement certification is required for this item in accordance with the requirements of 29 CFR 1903.19(c).
