29 CFR 1926.100(a): Employees were not protected by protective helmets while working in areas where there was a possible danger of head injury from impact, or from falling or flying objects, or from electrical shock and burns:    a) On or about 04/30/13 CSHO observed two employee working in an excavation in close proximity to excavator while not wearing any head protection, i.e. hard hats.    The Lucas Brother Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard CFR 1926.100(a), which was contained in OSHA inspection number 313985707, citation number 1, item number 1 and was affirmed as a final order on 05/12/2011, with respect to a workplace located at South Washington Street, Piscataway, NJ 08854.

Excavation was not made entirely in stable rock and exceeded 5 foot in depth:    a) On or about 04/30/2013 CSHO observed two men working in an excavation in excess of 5 foot deep with no protective system in place.     Lucas Brothers Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard CFR 1926.652(a)(1), which was contained in OSHA inspection number 313985707, citation 1, item 2 and was affirmed as a final order on 05/12/2011, with respect to a workplace located at South Washington Street, Piscataway, NJ 08854.
