North Jobsite - On or about May 01, 2013 employees were working from the second tier of a wooden scaffold plank that was 11 inches wide. The employees were exposed to a fall height of 15 feet.

29 CFR 1926.451(b)(3): Except as provided in paragraphs (b)(3)(i) and (ii) of this section, the front edge of all platforms shall not be more than 14 inches (36 cm) from the face of the work, unless guardrail systems are erected along the front edge and/or personal fall arrest systems are used in accordance with paragraph (g) of this section to protect employees from falling.  North Jobsite - On or about May 01, 2013 employees were working from the second tier of a wooden scaffold and the front edge of the platform was 17 1/2 inches from the face of the work. The employees were exposed to a fall height of 15 feet.

29 CFR 1926.451(e)(1): When scaffold platforms are more than 2 feet (0.6 m) above or below a point of access, portable ladders, hook-on ladders, attachable ladders, stair towers (scaffold stairways/towers), stairway-type ladders (such as ladder stands), ramps, walkways, integral prefabricated scaffold access, or direct access from another scaffold, structure, personnel hoist, or similar surface shall be used. Crossbraces shall not be used as a means of access.    North Jobsite - On or about May 01, 2013 employee climbed the scaffold crossbraces to access the second tier of a wooden scaffold. The employee was exposed to a fall height of 15 feet.

29 CFR 1926.451(g)(1): Each employee on a scaffold more than 10 feet (3.1 m) above a lower level shall be protected from falling to that lower level. Paragraphs (g)(1)(i) through (vii) of this section establish the types of fall protection to be provided to the employees on each type of scaffold. Paragraph (g)(2) of this section addresses fall protection for scaffold erectors and dismantlers.   Note to paragraph (g)(1): The fall protection requirements for employees installing suspension scaffold support systems on floors, roofs, and other elevated surfaces are set forth in subpart M of this part.     North Jobsite - On or about May 01, 2013 employees were installing insulation and siding using a wooden scaffold without fall protection. The employees were exposed to a fall height of 15 feet.
