29 CFR 1926.501(b)(13): Each employee engaged in residential construction activities 6 feet (1.8 m) or more above lower levels was not protected by guardrail systems, safety net system, or personal fall arrest system unless another provision in paragraph (b) of this section provides for an alternative fall protection measure.    a)  LaVern Miller DBA Pine Ridge Construction on or about May 1, 2013, at the address of 109 Chantilly Rue, Northwood, OH: The employer did not ensure employees were protected from falls while removing old and installing new roofing material to a residential structure.  Employees working in this condition were exposed to falls of up to 19 ft. to the ground below.

29 CFR 1926.503(a)(1): The employer did not provide a training program for each employee who might be exposed to fall hazards. The program shall enable each employee to recognize the hazards of falling and shall train each employee in the procedures to be followed in order to minimize these hazards.   a)  LaVern Miller DBA Pine Ridge Construction on or about May 1, 2013, at the address of 109 Chantilly Rue, Northwood, OH: The employer did not ensure that employees had been trained in the appropriate use of fall protection such as but not limited to the use of personal fall arrest systems or alternative measures while performing various residential roofing operations.
