29 CFR 1926.501(b)(4)(i): Each employee on walking/working surfaces was not protected from falling through holes (including skylights), more than six feet (1.8 m) above lower levels, by personal fall arrest systems, covers, or guardrail systems erected around such holes:    JOBSITE:  The employees, performing activities such as (but not limited to) operating a manual pulley to hoist cement to the platform of a scaffold for tuck pointing a chimney, and climbing the ladder frame to access the working platform of the scaffold during the course of masonry activities, were walking/working in close proximity where they were exposed to the hazard of falling 9 feet 1 inch to the ground through a hole which was not protected by guardrails, a cover or other equivalent means.    The RVM Construction LLC was previously cited for a violation of an equivalent Occupational Safety and Health standard 29 CFR 1926.501(b)(13), which was contained in OSHA inspection number 523958, citation number 01, item number 001a and was affirmed as a final order on 10/02/2012, with respect to a workplace located at 40 Wakemore Street, Darien, CT 06820.
