29 CFR 1910.22(a)(1): All places of employment, passageways, storerooms or service rooms were not kept clean and orderly or in a sanitary condition. a) for the work area containing equipment such as, but not limited to: the CNC cutting machine, sanding machines, edger, and horizontal boring machine, as observed on or about 05/03/2013. The local exhaust system was not capturing all the generated dust from cutting and sanding plywood and medium-density-fiber board (MDF Green Board). Accumulations of more than 1/4-inch of dust was measured on the horizontal surfaces such as the duct work, conduits and door frames. Employees were exposed to fire/flame-wave and explosion/deflagration hazards associated with the accumulation of combustible wood dust from the woodworking equipment. The dust was analyzed to be a Class II dust with an explosion severity of 2.11 and a pressure ratio of 8.18.

29 CFR 1910.22(a)(2): The floor of every workroom shall be maintained in a clean and, so far as possible, a dry condition. a) for the floor areas throughout the area containing equipment such as, but not limited to: the CNC cutting machine, sanding machines, edger, and horizontal boring machine, as observed on or about 05/03/2013. Large accumulations of dust on the floor were present around the machining equipment and the dust collection system. 29 CFR 1910.23(c)(1): Open-sided floors and/or platforms four feet or more above adjacent floor or ground level were not guarded with standard railings (or equivalent) and toeboards:Note: Toeboards are only required when persons can pass, there is moving equipment or there is equipment with which falling materials could create a hazard. for the loading dock area at the southeast corner of the facility, as observed on as noted on or about 05/03/2013. a) The open, upper edge of the dock had no chains or railings to prevent employeess from falling four-feet into the bottom of the ramp. b) The guard rails along the southeast edge of the ramp were rusted through and deteriorated so bad, they would not support the weight of an employee and allow them to fall 4-feet into the ramp, or be impaled by the exposed rebar holding part of the rail up.

In that the covered actuator pedal was used in a manner that allowed the operator to be exposed to the unguarded drill bits.

Employees had not been trained, medically qualified, or fit-tested to wear respirators. a) for the employee using a half-face, elastomeric respirator with organic vapor cartridges while spraying WilsonArt 801 contact adhesive. One of the respirators had no bottom strap, no inhalation valve covers and the charcoal was coming out of the cartridge. b) for employees throughout the facility using filtering face-piece dust masks while working in the cutting / sanding department.
