29 CFR 1926.304(i)(1): Each circular hand-fed rip saw was not guarded by a hood which completely enclosed that portion of the saw blade above the table and that portion of the saw above the material being cut. On April 30, 2013 an employee was exposed to an amputation as the employee used a table saw that was missing the guard that covers the blade section that was above the table top.

29 CFR 1926.451(g)(1)(i): Each employee on a boatswains' chair, catenary scaffold, float scaffold, needle beam scaffold, or ladder jack scaffold was not protected by a personal fall arrest system: On April 30, 2013 employees were not protected by a personal fall arrest system while working from a ladder jack scaffold, exposing employees to a fall hazard of 15.9 feet above grade. Abatement documentation is required for this item in accordance with the requirements of 29 CFR 1903.19(d).
