29 CFR 1910.147(c)(8): Lockout or tagout was not performed only by the authorized employees who are performing the servicing or maintenance: The employer does not ensure that lockout or tagout is performed by the authorized employee who is performing the service or maintenance. This violation occurred on or about May 16, 2013, in the cutting area where a saw operator was exposed to struck-by and caught-in hazards when performing service and maintenance on the Oster 716 without applying a lock to the disconnect. Pursuant to 29 C.F. R. 1903.19, within ten (10) calendar days of the abatement date, the employer must submit documentation showing that it is in compliance with the standard, including describing the steps that it is taking to ensure that lockout or tagout is performed by the authorized employee who is performing the service or maintnance.

This violation occurred on or about May 16, 2013, in Weld Cell #3 where a welder was exposed to electrical hazards when the cable to the foot pedal of the Miller Syncrowave 250 DX was cut exposing a splice. Pursuant to 29 C.F. R. 1903.19, within ten (10) calendar days of the abatement date, the employer must submit documentation showing that it is in compliance with the standard, including describing the steps that it is taking to ensure that cables with damaged insulation or exposed bare conductors are replaced.
