29 CFR 1910.22(a)(1): All places of employment, passageways, storerooms or service rooms were not kept clean and orderly or in a sanitary condition. a) Clybourn Metal Finishing Co., Polishing and Buffing Rooms: The area where aluminum polishing and buffing were performed had explosive dust on top of the buffing and polishing machines, on top of the ductwork, on top of a locker and in and around the machinery. Among other methods, a feasible method to correct this hazard would be to comply with National Fire Protection Association (NFPA) Standard 654, 2013 Edition, "Standard for the Prevention of Fire and Dust explosions from the Manufacturing, Processing, and Handling of Combustible Particulate Solids", Chapter 8, Section 8.1.2 - The dust shall be conveyed to air material separators; and, Section 8.2.1 - Cleaning Frequency. In accordance with 29 CFR 1903.19(c), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET).

29 CFR 1910.134(c)(2)(i): Respirator users were not provided with the information contained in Appendix D to 29 CFR 1910.134 when the employer determined that any voluntary respirator use was permissible: a) Clybourn Metal Finishing Co., Polishing and Buffing Rooms: Employees wore 3M Model 8210 filtering facepiece respirators and were not provided with the information contained in Appendix D to 29 CFR 1910.134 when employees used the respirators voluntarily during polishing and buffing of aluminum parts. In accordance with 29 CFR 1903.19(c), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET).

Employees wore 3M Model 8210, N95 filtering facepiece respirators with the lower strap removed during polishing and buffing of aluminum parts. In accordance with 29 CFR 1903.19(c), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET).
