29 CFR 1926.451(b)(1)(i): Each platform unit (e.g., scaffold plank, fabricated plank, fabricated deck, or fabricated platform) was not installed so that the space between adjacent units and the space between the platform and the uprights is no more than 1 inch (2.5 cm) wide. The employer could not demonstrate that a wider space was necessary (for example, to fit around uprights when side brackets are used to extend the width of the platform): a)Troy Community Hospital, Troy, PA: The scaffold platform was constructed with three 9 1/2 inch wide planks on frames that were 34 inches wide leaving gaps of 4 1/2 to 5 1/2 inches exposing workers to trip and fall hazards , on or about May 2, 2013. ABATEMENT CERTIFICATION REQUIRED
