29 CFR 1910.1030(f)(2)(i):  Hepatitis B vaccination was not made available after the employee had received the training required in 29 CFR 1910.1030(g)(2)(vii)(I):   a) During the period of approximately February 1, 2013 and May 1, 2013, at the above located workplace, new employees having occupational exposure to blood or other potentially infectious materials were offered the Hepatitis B vaccination without the training required in 29 CFR 1910.1030(g)(2)(vii)(I). Therefore, the employees were not provided with the information and knowledge to understand the benefits of the Hepatitis B vaccination series.

29 CFR 1910.1030(g)(2)(ii)(A):  The employer did not ensure that training was provided to employees with occupational exposure at the time of initial assignment to tasks where occupational exposure might take place:    a) During the period of approximately February 1, 2013 and May 1, 2013, at the above located workplace, new employees were required to perform duties having occupational exposure to blood or other potentially infectious materials. The employees were not provided training (as required by 1910.1030(g)(2)) at the time of initial assignment to tasks where occupational exposure to bloodborne pathogens might occur.

29 CFR 1910.1030(c)(1)(iv):   The Exposure Control Plan was not reviewed and updated at least annually and whenever necessary to reflect new or modified task and procedures which affect occupational exposure and reflect new or revised employee positions with occupational exposure. The review and update of such plan did not: i) Reflect changes in technology that eliminate or reduce exposure to bloodborne pathogens; and ii) Document annually considerations and implementations of appropriate commercially available and effective safer medical devices designed to eliminate or minimize   occupational exposure.  a) On or about May 1, 2013, at the above located workplace, where employees were exposed to blood and other potentially infectious material, the workplace bloodborne pathogen exposure control program had not been reviewed with written documentation since 2006.
