29 CFR 1926.102(a)(1): Eye and face protective equipment were not used when machines or operations presented potential eye or face injury from physical, chemical, or radiation agents.  (a) On the second level, new construction located at 79 Canyon View Dirve, Newnan, Ga.:  An employee using a pneumatic Hitachi (Model NV83A2S1) Strip Nail Gun was not protected from eye injuries while installing roof trusses with 3-inch round head plastic collated framing nails.

29 CFR 1926.404(f)(6): The path to ground from circuits, equipment, or enclosures was not permanent and continuous:    (a) Front, new residential construction located at 79 Canyon View Drive, Newnan, Ga.: Employees used an orange 100-foot extension cord that was missing a ground pin. This damaged cord was used to power a double insulated Makita (Model - 5007F) 7 1/4-inch circular saw.  The cord was over wet conditions, exposing employees to a shock hazard.

29 CFR 1926.451(b)(2): Except as provided in paragraphs (b)(2)(i) and (b)(2)(ii) of this section, each scaffold platform and/or walkway was not at least 18 inches (46 cm) wide:  (a) Top of second level, new residential construction located at 79 Canyon View Drive, Newnan, Ga.: Employees framing roof trusses at heights of approximately 25-feet were exposed to falls to lower levels while using a 2x6 as a makeshift scaffold platform which was approximately 5 1/2 inches wide.

29 CFR 1926.451(g)(1)(vii): Each employee on a scaffold, not otherwise specified in paragraphs (g)(1)(i) through (g)(1)(vi) of this section, more than 10 feet (3.1 m) above lower level was not protected from falls by the use of personal fall arrest systems or guardrail systems meeting the requirements of paragraph (g)(4) of this section.  (a) Front, new construction located at 79 Canyon View Drive, Newnan, Ga.: Employees using an elevated scaffold work platform to access the roof at heights of approximately 25-feet were not protected from falls to the ground level.  Employees using the scaffold platform were not provided with guardrails or an anchor point to attach their personal fall protection systems.

29 CFR 1926.1053(b)(1): Portable ladders were used for access to an upper landing surface and the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface to which the ladder was used to gain access:  (a) Front, first floor, new residential construction located at 79 Canyon View Drive, Newnan, Ga.: The ladder provided was not secured to the structure and did not extend 3 feet above the working surface to allow for safe access on and off the roof. Employees accessing this ladder were exposed to falls of approximately 10 feet 2 inches to a lower level.
