Debris from the forging process is piled up around the machines, observed on or about 4/19/13.    b) Machine Shop: The area around the HEM band saw was not kept clean and orderly while employees were working in this area. Debris from the forging of metals and rags were piled up around the machines, observed on or about 4/19/13.    c) Machine Shop: The area around Machine 421 was not kept clean and orderly while employees were working in this area. Debris from the forging of metals is piled up around the machines, observed on or about 4/19/13.

29 CFR 1910.147(c)(4)(i): Procedures were not developed, documented and utilized for the control of potentially hazardous energy when employees were engaged in activities covered by this section:    (a) Workplace:  The employer did not develop, document and utilize procedures for the control of potentially hazardous energy while employees were engaged in service and maintenance activities of lathes, CNC machines, presses, saws and other machines, observed on or about 4/19/13.    ABATEMENT NOTE:    The procedures should clearly and specifically outline the scope, purpose, authorization, rules and techniques to be utilized for the control of hazardous energy, and the means to enforce compliance including but not limited to:    A specific statement of the intended use of the procedure    Specific procedural steps for shutting down, isolating, blocking and servicing machines or equipment to control hazardous energy.     Specific procedural steps for the placement, removal and transfer of lockout devices or tagout devices and the responsibility for them.     Specific requirements for testing a machine or equipment to determine and verify the effectiveness of lockout devices, tagout devices and other energy control measures.
