29 CFR 1926.1053(b)(16): Portable ladders with structural defects, such as, but not limited to, broken or missing rungs, cleats, or steps, broken or split rails, corroded components, or other faulty or defective components, were not either immediately marked in a manner that readily identified them as defective, or tagged with Do Not Use or similar language, and were not withdrawn from service until repaired:      a) On or about 05/03/13 CSHO observed two employees use a portable aluminum ladder to access the roof of a one story, single family home when the ladder was damaged. i.e. side rails bent/twisted and not tagged "Do Not Use" and or taken out of service.

Segundo Construction aka Segundo Quality Construction was previously cited for a violation of this occupational safety and health standard or its equivalent standard CFR 1926.102(a)(1) which was contained in OSHA inspection number 311922496, citation number 1, item number 1 and was affirmed as a final order on 12/11/09, with respect to a workplace located at 521-525 Amboy Avenue, Woodbridge, NJ 07095.

The portable ladder was not secured at its top to a rigid support, and a grasping device, such as a grabrail, was not provided to assist employees in mounting and dismounting the ladder:    a) On or about 05/03/13 CSHO observed two employees use a portable extension ladder to access the roof of a one story, single family home when the ladder did not extend three feet above the landing.    Segundo Construction aka Segundo Quality Construction was previously cited for a violation of this occupational safety and health standard or its equivalent standard CFR 1926.1053(b)(1), which was contained in OSHA inspection number 311922496, citation number 2, item number 1 and was affirmed as a final order on 12/11/09, with respect to a workplace located at 521-525 Amboy Avenue, Woodbridge, NJ 07095.
