29 CFR 1926.453(b)(2)(v): A body belt was not worn and/or a lanyard was not attached to the boom or basket when working from an aerial lift: a) 7001 Route 309, Coopersburg, PA., Exterior East Wall, Second Level -- On or about May 2, 2013, an employee operating an extensible boom aerial lift while wearing a personal fall arrest harness and lanyard that was not attached to the aerial lift's anchorage point, exposing the employee to a fall hazard of no less than 14 feet.
