29 CFR 1910.147(c)(1): The employer did not establish a program consisting of an energy control procedure, employee training and periodic inspections to ensure that before any employee performed any servicing or maintenance on a machine or equipment where the unexpected energing, startup or release of stored energy could occur and cause injury, the machine or equipment shall be isolated from the energy source and rendered inoperative:     River to River Residential Corporation dba Hurricane Creek Market, 400 Lou Ann Drive, Herrin, IL 62948:      The employer did not implement a written lockout-tagout program outlining the steps required to de-energize equipment.  Employees were subject to electrical shock when changing lighting ballasts during the last 4 months due to unexpected energization of equipment due to improper lockout.  Employees who were not lockout Authorized had access to keys for the breaker panel closet.

Employees in the kitchen operating equipment plugged into the power tap were subject to electric shock hazards and fire hazards.

Employees were exposed to electric shock and fire hazards when the area is wet and they stand near it and operate electric kitchen equipments that is plugged into the power tap and the receptacle.
