29 CFR 1910.67(c)(2)(v): A body belt was not worn and a lanyard attached to the boom or basket when working from an aerial lift:    On or about May 3, 2013, an employee was observed working from an elevated work platform of an aerial lift without a body harness and lanyard attached to the aerial lift.  Employee was performing electrical work on a commercial business sign.  Employee was exposed to a fall hazard of approximately 25 feet to the parking lot ground below.
