29 CFR 1910.147(c)(1): Energy control program. The employer shall establish a program consisting of energy control procedures, employee training and periodic inspections to ensure that before any employee performs any servicing or maintenance on a machine or equipment where the unexpected energizing, startup or release of stored energy could occur and cause injury, the machine or equipment shall be isolated from the energy source and rendered inoperative:     (a)  There was no program on energy control procedures, employee training, and periodic inspections, including requiring employees to lock out the mechanical power presses when cleaning and replacing dies working on the dies in the presses, locking out the injection molding machines when changing molds, locking out the saws when doing blade changes, and locking out the robotic cells connected to the arc welders when sweeping out the robotic cells and cleaning the fixtures on the robotic arms.      In accordance with 29 CFR 1903.19(d), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET), and in addition, documentation demonstrating that abatement is complete must be included with your certification.  This documentation may include, but is not limited to, evidence of the purchase or repair of the equipment, photographic or video evidence, or written records.

29 CFR 1910.212(a)(1):  Types of guarding. One or more methods of machine guarding shall be provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks. Examples of guarding methods are-barrier guards, two-hand tripping devices, electronic safety devices, etc.:     (a)  Employees operating the DoAll Vertical Band Saw, were exposed to 1-1/2 inches of guarded blade.    (b)  Employees operating the Takisawa Machine Lathe were exposed to the unguarded chuck.      In accordance with 29 CFR 1903.19(d), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET), and in addition, documentation demonstrating that abatement is complete must be included with your certification. 29 CFR 1910.217(c)(1)(i): It shall be the responsibility of the employer to provide and insure the usage of "point of operation guards" or properly applied and adjusted point of operation devices on every operation performed on a mechanical power press. See Table O-10:    (a)  The backs of the Aida Mechanical Power Press (S/N 10530-6042) and Aida Mechanical Power Press (S/N C0520-0107) were not guarded, thereby exposing employees to the point of operation.      In accordance with 29 CFR 1903.19(d), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET), and in addition, documentation demonstrating that abatement is complete must be included with your certification. The employer shall maintain a certification record of inspections which includes the date of inspection, the signature of the person who performed the inspection and the serial number, or other identifier, of the power press that was inspected:    (a)  There were no periodic and regular inspections of the partial revolution mechanical power presses.
