29 CFR 1926.102(a)(1): Eye and face protective equipment were not used when machines or operations presented potential eye or face injury from physical, chemical, or radiation agents.     (a) 2227 Knoxhill View SE, Smyrna, GA. - On the front side, second floor of a multi-family housing structure, an employee was not protected from eye injuries while framing with a pneumatic Hitachi Model-NRAB2 (S1) Strip Gun, as this tool was operated without safety glasses.

29 CFR 1926.501(b)(13): Each employee engaged in residential construction activities 6 feet (1.8 m)  or more above lower levels was not protected by guardrail systems, safety net systems, personal fall arrest systems, or an alternative fall protection measure under another provision of paragraph 1926.501 (b), nor did the employer demonstrate that it is infeasible or creates a greater hazard to use these systems and develop and implement a fall protection plan which meets the requirements of paragraph (k) of section 1926.502:     (a) 2227 Knoxhill View SE, Smyrna, Ga.: Employees were exposed to fall hazards of approximately 18 feet, 8-inches to the ground while engaged in framing activities on 2nd level of new construction without fall protection in place on 3/28/2013.    (b) 2227 Knoxhill View SE, Smyrna, Ga.: An employee was exposed to a fall hazard of approximately 18 feet, 8 inches to the ground while exgaged in framing activities on 2nd level of new construction without fall protection in place on 3/29/2013.      (c) 2227 Knoxhill View SE, Smyrna, Ga.: An employee was exposed to a fall hazard of approximately 20 feet above ground level on a new residential construction; while sheating roof of new constrction multi-family site on 04/08/2013.

29 CFR 1926.503(a)(1): The employer did not provide a training program for each employee potentially exposed to fall hazards to enable each employee to recognize the hazards of falling and the procedures to be followed in order to minimize these hazards:     (a) 2227 Knoxhill View SE, Smyrna, Ga.: Employer did not provide fall protection training to employees working performing framing and sheathing activities on a 9/12 pitched roof at heights of approximately 18 feet, 8 inches to the ground.  The employees were wearing no fall protection.
