29 CFR 1910.334(a)(2)(i): Portable cord and plug connected equipment and flexible cord sets (extension cords) were not visually inspected before use on any shift for external defects (such as loose parts, deformed and missing pins, or damage to outer jacket or insulation) and for evidence of possible internal damage (such as pinched or crushed outer jacket): a) Max Chop Saw - Employees were operating an electric Chop Saw powered by a cord with highly noticeable outer jacket damage that had not been visually inspected and removed from service before its use, on or about 4/24/13.

29 CFR 1910.1200(f)(5): The employer did not ensure that each container of hazardous chemicals in the workplace was labeled, tagged, or marked with the identity and appropriate hazard warnings regarding the chemicals contained therein: a) Outside Rear Door and Inside Facility - MacMillan cleaning fluid is being transferred from a 55 gallon drum into paint cans that were not labeled to identify the container's contents, on or about 4/24/13.
