29 CFR 1926.20(b)(2): The employer did not initiate and maintain a safety program which provides for frequent and regular inspections of jobsites, materials, and equipment to be made by a competent person.    Workers were engaged in residential construction activities re-roofing an existing single-family house. Workers were exposed to potential electrocution and fall hazards in that  regular and frequent inspections by a competent person was not conducted to ensure that the job site, materials, and equipment such as electrical service and fall protection systems were maintained and utilized in a safe manner.    In accordance with 29 CFR 1903.19(d), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET), and in addition, documentation demonstrating that abatement is complete must be included with your certification.  This documentation may include, but is not limited to, evidence of the purchase or repair of the equipment, photographic or video evidence, or written records.

29 CFR 1926.503(a)(1): The employer did not provide a training program for each employee potentially exposed to fall hazards to enable each employee to recognize the hazards of falling and the procedures to be followed in order to minimize these hazards:     Workers were observed engaged in residential construction activities re-roofing an existing single-family house. Workers were exposed to potential 23.7 foot fall hazards in that workers were not properly trained in fall protection criteria.    In accordance with 29 CFR 1903.19(d), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET), and in addition, documentation demonstrating that abatement is complete must be included with your certification.  This documentation may include, but is not limited to, evidence of the purchase or repair of the equipment, photographic or video evidence, or written records.

29 CFR 1926.404(b)(1)(ii): On a construction site, where an assured equipment grounding program was not utilized, all 120-volt, single-phase, 15 and 20 ampere receptacle outlets which were not a part of the permanent wiring of the building or structure and which are in use by employees did not have approved ground fault circuit interrupers for personal protection.    Workers were engaged in residential construction activities re-roofing an existing single-family house. Located along the side area of the house worker(s) utilized a DeWalt circular saw that was connected to the electrical service with a orange  50.0 foot extension cord. Worker(s) were exposed to potential electrocution hazards in that the electrical service was not protected with a ground-fault circuit interrupter and an assured equipment grounding conductor program was not in place.

29 CFR 1926.404(f)(6): The path to ground from circuits, equipment, or enclosures was not permanent and continuous:   Workers were engaged in residential construction activities re-roofing an existing single-family house. Located along the side area of the house worker(s) utilized a DeWalt circular saw that was connected to the electrical service with a 50 foot extension cord. Worker(s) were exposed to potential electrocution hazards in that the extension cord was missing a grounding pin.

Located the upper roof area workers were exposed to potential 23.7 foot fall hazards in that a proper fall protection system was not utilized.
