29 CFR 1926.100(a): Employees working in areas where there was a possible danger of head injury from impact, or falling or flying objects, or from electrical shock and burns, were not protected by protective helmets:  On or about May 04, 2013 the employer did not ensure employees working on the ground were protected by a hard hat while other employees worked above. The employees on the ground were exposed to being struck-by objects falling from a height of over 15 feet.

29 CFR 1926.102(a)(1): Employees were not provided with eye and face protection equipment when machines or operations present potential eye or face injury from physical, chemical, or radiation agents.  On or about May 04, 2013, employees using an electric circular saw to cut and trim wood were not provided with eye protection exposing them to eye injuries.

29 CFR 1926.501(b)(13):  Each employee engaged in residential construction activities 6 feet (1.8 m) or more above lower levels shall be protected by guardrail systems, safety net system, or personal fall arrest system unless another provision in paragraph (b) of this section provides for an alternative fall protection measure.   On or about May 04, 2013, the employer did not ensure employees performing framing work at a height of more than 6 feet were protected from falling. The employees were exposed to an unprotected fall to the ground below.

29 CFR 1926.503(a)(1): The employer shall provide a training program for each employee who might be exposed to fall hazards. The program shall enable each employee to recognize the hazards of falling and shall train each employee in the procedures to be followed in order to minimize these hazards.  On or about May 04, 2013, the employer did not ensure employees exposed to fall hazards were trained to recognize and avoid such hazards. The employees working without fall protection were exposed to an unprotected fall of over 15 feet to the ground below.

29 CFR 1926.1051(a): Stairway(s) or ladder(s) were not provided at all personnel points of access where there was a break in elevation of 19 inches (48 cm) or more, or no ramp, runway, sloped embankment, or personnel hoist was provided:  On or about May 04, 2013, employees who were performing framing activities were not provided with a ladder for access and/or egress from the upper working working levels and climbed the interior wall framing.
