29 CFR 1926.102(a)(1): Eye and face protective equipment were not used when machines or operations presented potential eye or face injury from physical, chemical, or radiation agents.   (a) 206 Elliott Way, Smyrna, GA. - On the front side, second floor of a single-family housing structure, employees were not protected from eye injuries while framing with a pneumatic Hitachi Model-NRAB2 (S1) Strip Gun, as this tool was operated without safety glasses.

29 CFR 1926.501(b)(13): Each employee engaged in residential construction activities 6 feet (1.8 m)  or more above lower levels was not protected by guardrail systems, safety net systems, personal fall arrest systems, or an alternative fall protection measure under another provision of paragraph 1926.501 (b), nor did the employer demonstrate that it is infeasible or creates a greater hazard to use these systems and develop and implement a fall protection plan which meets the requirements of paragraph (k) of section 1926.502:   (a) 206 Ellliott Way, Smyrna, Ga.: Employees were exposed to a fall hazard of approximately 13 feet to the ground while engaged in framing activities on the 2nd level of new construction without fall protection in place on 05/02/2013.

29 CFR 1926.503(a)(1): The employer did not provide a training program for each employee potentially exposed to fall hazards to enable each employee to recognize the hazards of falling and the procedures to be followed in order to minimize these hazards:   (a) 206 Elliott Way, Smyrna, Ga.: Employer did not provide fall protection training to employees working performing framing activities at height of approximately 13 feet to the ground.  The employees were wearing no fall protection.
