29 CFR 1910.23(c)(1):  Every open-sided floor or platform 4 feet or more above adjacent floor or ground level was not guarded by a standard railing (or the equivalent as specified in paragraph (e)(3) of this section) on all open sides:    a.  on top of oilfield waste fluid holding tanks, employees overseeing truck unloading operations were exposed to a fall of 8 feet or greater to the ground while working without fall protection    b.  along the storage tank middle corridor, employees overseeing the holding frac tanks were exposed to a fall of over 6 feet to ground on fixed walkways where guardrail sections were not provided with a mid rail    c.  at the generator shed (rental trailer), employees performing maintenance with an open door and no stairway were exposed to a fall hazard of over 4 feet to ground working without fall protection.
