29 CFR 1926.760(a)(1): Each employee engaged in a steel erection activity who is on a walking/working surface with an unprotected side or edge more than 15 feet (4.6 m) above a lower level was not protected from fall hazards by guardrail systems, safety net systems, personal fall arrest systems, positioning device systems, or fall restraint systems. a) Worksite: An employee engaged in the erection of steel panels on a roof was wearing a loose fitting safety harness and was not fully protected from falls of between 32 and 40 feet.

29 CFR 1926.503(c)(3): Inadequacies in an affected employee's knowledge or use of fall protection systems or equipment indicate that the employee has not retained the requisite understanding or skill. a) Worksite: An employee working at the site was wearing a safety harness which was too large to fit snuggly, and neither the employee nor the employer representative on site seemed to have retained that knowledge that a proper fitting harness is needed for it to properly protect the employee in case of a fall. ABATEMENT CERTIFICATION IS REQUIRED
