Maintenance department had been obtaining hydrogen peroxide exposure readings while trouble shooting employee health complaints but did not maintain exposure records.

Maintenance personnel had periodically recorded hydrogen peroxide levels upon employee complaint of illness but records were not maintained and not provided during inspection.
