29 CFR 1926.451(e)(9)(i): The employer did not provide safe means of access for each employee erecting or dismantling a scaffold where the provision of safe access was feasible and did not create a greater hazard: a) Jobsite, 416 7th Avenue, Pittsburgh, PA: On or about May 2, 2013, during scaffold re-decking activities, the employer did not ensure that employees used a safe means of getting up and down from the scaffold decking. An employee had a portable ladder in place to get up/down from the scaffolding, but it was extended approximately 6-12" below the scaffold decking level.
