29 CFR 1926.403(b)(1): Employer did not ensure that electrical equipment is free from recognized hazards that are likely to cause death or serious physical harm to employees: On or about May 6, 2013, in front the building, approximately 15-feet in the air, a 100-foot orange extension cord (125V nominal) used to power the angle grinder used by the employee removing insulation from the cement wall had outer insulation damage in three separate locations along the cord set. The employee(s) was exposed to electrical shock hazards.
