Employees were working on the side of building roof, these employees were performing roofing operations on a roof of 4-12 pitch with an eave height of 22 feet without fall protection in place exposing employees to fall hazards

John Papalardo, and Ken Richards from Frankoski Construction, East Orange NJ site. These employees were utilizing a portable ladder that was not secured and did not extend 3 feet above the working surface of the roof to allow for safe access on and off the roof.
