On or about May 7, 2013, employees working on the rigs while logging for the company are required to provide their own personal protective equipment (PPE) that include fire resistant clothing (FRC), steel toes shoes, and hard hats.

On or about May 7, 2013, the employer did not ensure that training was provided to the employees on the hazard communication program, exposing the employees to inhalation and absorption hazards while working with chemicals.
