29 CFR 1926.20(b)(2): The employer did not ensure that frequent and regular inspections of the job sites, materials, and equipment were conducted by a competent person:    On or about May 8, 2013, at the above addressed jobsite, employer did not perform inspections of the jobsite including, but not limited to, the work area on the second floor where an employee was going to perform air conditioner duct work installation, exposing the employee to a fall hazard.       NOTE: CERTIFICATION OF ABATEMENT MUST BE SUBMITTED FOR THIS ITEM.

29 CFR 1926.501(b)(14): Each employee working on, at, above, or near wall openings (including those with chutes attached), where the outside bottom edge of the wall opening was 6 feet (1.8 m) or more above lower levels and the inside bottom edge of the wall opening is less than 39 inches (1.0 M) above the walking/working surface, is not protected from falling by the use of a guardrail system, a safety net system, or a personal fall arrest system.    On or about May 8, 2013, at the above addressed jobsite, an employee was engaged in the installation of air conditioning duct work on a second floor at approximately 10 feet 6 inches above ground level from a step ladder that was positioned approximately 3 feet from a staircase opening with no guardrails, exposing him to a fall hazard.       NOTE: CERTIFICATION AND DOCUMENTATION OF ABATEMENT MUST BE SUBMITTED FOR THIS ITEM.
