29 CFR 1910.178(l)(1)(i): The employer did not ensure that each powered industrial truck operator is competent to operate a powered industrial truck safely, as demonstrated by the successful completion of the training and evaluation specified in this paragraph (l): (a ) On May 1st, 2013 at Collins Machinery & Engineering: Employees were exposed to struck by and crushing hazards when it was discovered during inspection the employer had not established a training program for employees required to operate powered industrial trucks (PIts).

29 CFR 1910.303(b)(1): Electrical equipment was not free from recognized hazards that were likely to cause death or serious physical harm to employees: (a) On May 1st, 2013 Collins Machinery & Manufacturing: Employees were exposed to contact with electrical hazards when the insulation on several welding leads was removed or pulled away from the connecting point of the plug. Internal wiring was exposed to employees handling the welding leads.
