Abatement certification required within 10 days after abatement date. The certification shall include a statement that abatement is complete, date and method of abatement, and states employees and their representatives were informed of this abatement.

29 CFR 1926.451(b)(3): The front edge of all platforms was not 14 inches (36 cm) or less from the face of the work, and no guardrail systems were erected along the front edge and/or no personal fall arrest systems were used in accordance with paragraph (g) of this section to protect employees from falling: a) North Side of the Building - A laborer was observed straddling between a tubular welded frame scaffold and the window sill which were 30 inches apart exposing him to a 51 inch fall on to the porch overhang, on or about 05/06/13. Abatement certification required within 10 days after abatement date. The certification shall include a statement that abatement is complete, date and method of abatement, and states employees and their representatives were informed of this abatement.

29 CFR 1926.451(e)(1): When scaffold platforms were more than 2 feet (0.6 m) above or below a point of access, portable ladders, hook-on ladders, attachable ladders, stair towers (scaffold stairways/towers), stairway-type ladders (such as ladder stands), ramps, walkways, integral prefabricated scaffold access, or direct access from other scaffold, structure, personnel hoist, or similar surface was not used. Cross-braces were used as a means of access. a) North Side of the Building - A laborer were observed on a tubular welded frame scaffold climbing the cross braces to reach the protective plastic sheet exposing them to a 50 inch fall onto the basement patio, on or about 05/06/13. b) South Side of the Building - Laborers were observed on a tubular welded frame scaffold climbing the cross braces to access the ground below exposing them to a 8 foot fall onto the basement patio, on or about 05/06/13. Abatement certification required within 10 days after abatement date. The certification shall include a statement that abatement is complete, date and method of abatement, and states employees and their representatives were informed of this abatement.

Abatement certification required within 10 days after abatement date. The certification shall include a statement that abatement is complete, date and method of abatement, and states employees and their representatives were informed of this abatement.
