29 CFR 1904.32(b)(2)(ii): The OSHA 300A Annual Summary did not include the calendar year covered, the company's name, establishment name, establishment address, annual average number of employees covered by the OSHA 300 Log, and/or the total hours worked by all employees covered by the OSHA 300 Log.  JOB SITE: 2200 E. Malone Avenue, Sikeston, MO 63801, the employer failed to accurately complete the OSHA 300A Annual Summary which did not include the correct average number of employees and total hours worked by all employees covered by the OSHA 300 Log for calendar years 2010, 2011, and 2012.
