29 CFR 1910.36(d)(1): Employee(s) were not able to open an exit route door from the inside at all times without keys, tools, or special knowledge: On or about 7 May 2013, the exit door of the conference room, locks from the inside automatically. The push button release is not located or positioned in a manner which is obvious to its function, exposing the employees to fire hazards.

29 CFR 1960.8(a): 29 CFR 1960.8(a): The Agency head did not furnish employment and a place of employment to each employee that were free from recognized hazards that caused or were likely to cause death or serious physical harm: On or about 7 May 2013, the employer did not ensure that training on the use of traffic control devices was performed with employees required to work near roadways, exposing employees to the hazard of being struck by motor vehicles. Among other methods, feasible abatement includes protection such as ensuring training is provided in the use of traffic control devices to reduce the hazards of working near motor vehicle traffic.
