29 CFR 1910.178(p)(1): Powered industrial truck(s) found to be in need of repair, defective, or in any way unsafe had not been  taken out of service until restored to safe operating condition(s):   Army Air Force Exchange - Davis Monthan AFB - On or about May 3, 2013 and at times prior thereto Powered Industrial Trucks were not removed from service until restored to a safe operating conditions for:   a) Caterpillar Model P4000 with Serial Number AT3503879 located at AAFES Bldg. 2527:   -  The right rear pneumatic tire was missing a 5-6 inch section of rubber in one location and smaller chunks in other locations that could lead to improper balance of the forklift on cornering and on rough surfaces, load shift when turning on corners and increased possibility of tire failure during operation by AAFES employees.   -  The left rear pneumatic tire of the P4000 contained smaller chunks of tire missing along the outer edge with one gash penetrating a third of the way through the tire that could lead to improper balance of the forklift on cornering and on rough surfaces, load shift when turning on corners and increased possibility of tire failure during operation by AAFES employees.   b)  Caterpillar Model P5000 with Serial Number AT3505277 located at the Housing Shopette Bldg. 6040:   - the right rear pneumatic tire was missing the outer section of rubber along 21 percent of it's outer edge with some sections missing down to the replacement line.  A short distance later another 7 percent of the outer edge of the tire was missing that could lead to improper balance of the forklift on cornering and on rough surfaces, load shift when turning on corners and increased possibility of tire failure during operation by AAFES employees.
