29 CFR 1910.147(c)(4)(ii)(D): The energy control procedures did not clearly and specifically outline the requirements for testing a machine or equipment to determine and verify the effectiveness of lockout devices, tagout devices, and other energy control measures. a) For the Newspaper Printing Company facility at 5210 South Lois Ave., Tampa, FL 33635, as observed on or about 05/09/2013, machine specific lockout tagout procedures in place did not require that all energy sources be tested prior to use, in order to ensure that all energy sources had been isolated/de-energized. Equipment on which maintenance was performed utilized 240/480V electricity and compressed air as hazardous energy.

29 CFR 1910.147(c)(7)(i)(A): Authorized employee(s) did not receive training in the recognition of applicable hazardous energy sources, the type and magnitude of the energy available in the workplace, and the methods and means necessary for energy isolation and control: a) For the Newspaper Printing Company facility at 5210 South Lois Ave., Tampa, FL 33635, as observed on or about 05/09/2013, all authorized maintenance employees were not aware that lockout tagout procedures were in place nor were they instructed on the use of the procedures. Written procedures were outdated in that they listed maintenance personnel who no longer worked at the facility, and did not list current maintenance employees who worked at the facility as authorized employees. Hazardous energy was present on printing presses and other equipment in the form of 240/480V electricity and compressed air.

29 CFR 1910.215(b)(9): The distance between the abrasive wheel periphery(s) and the adjustable tongue or the end of the safety guard peripheral member at the top exceeded one fourth inch: a) For the Newspaper Printing Company facility at 5210 South Lois Ave., Tampa, FL 33635, as observed on or about 05/09/2013, a bench grinder in the maintenance shop was missing an adjustable tongue guard, and a ½ inch gap existed between the abrasive wheel and the peripheral wheel guard.
