29 CFR 1926.1053(b)(1): Where portable ladders were used for access to upper landing surfaces, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface to which the ladders were used to gain access.     On or about May 7, 2013 and times prior thereto, at the excavation site located approximately one block north of Main Road on Carley Road in Springdale, Arkansas, employees were exposed to fall hazards while getting in and out of the excavation.  The access ladder positioned on the side wall of the excavation did not extend above the landing surface.
