29 CFR 1926.502(f): Warning line systems [See 1926.501(b)(10)] and their use did not comply with the following provisions: 29 CFR 1926.502(f)(1) through 29 CFR 1926.502(f)(4): a) Angelos Roofing & Construction, Inc. / 1105 West 12th Street, Erie, PA: On or about May 9, 2013 employees were working on a flat roof: 1) The warning line was not erected around all sides of the work area 2) The warning line was not 6 feet or greater from the edge of the roof 3) Points of access, materials handling areas, storage areas, and hoisting areas were not connected to the work area by an access path formed with two warning lines. 4) The rope, wire, or chain was not supported in such a way that that sag was between 34 and 39 inches. 5) The use of a safety monitor as the only means of fall protection was on a roof with a width greater than 50 feet.

The systems were not installed and used as specified by regulations.
