Employees working on or near the active roadway did not have sufficient training to recognize and control the hazards of their work environment. Observed on or about 09 May 2013. Verde Electric Corp was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.21(b)(2):which was contained in OSHA inspection number 311284830, citation number 01, item number 002b and was affirmed as a final order on 8/13/2010, with respect to a workplace located at Route 119 in front of 155 White Plains Road.

29 CFR 1926.200(g)(2): Traffic control signs or devices used for protection of workers did not conform to Part VI of the Manual of Uniform Traffic Control Devices (MUTCD), 1988 Edition, Revision 3, September 3, 1993, FHWA-SA-94-027 or Part VI of the Manual on Uniform Traffic Control Devices, Millenium Edition, incorporated by reference in Sec. 1926.6. a) 115 South Ridge Street, Rye Brook, New York. On or about 09 May 2013. Employee(s) working from a lift, 14 feet above an active roadway, were not protected from being struck by vehicular traffic. Advance warning signs and traffic control devices did not conform to Part 6 TEMPORARY TRAFFIC CONTROL, Chapters 6B - 6H of the MUTCD 2000 edition. Only four cones were placed around the lift vehicle to warn and direct traffic. Verde Electric Corp was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.200(g)(2): which was contained in OSHA inspection number 311284830, citation number 01, item number 001a and was affirmed as a final order on 5/11/2008, with respect to a workplace located at Route 119 in front of 155 White Plains Road.
