29 CFR 1926.501(b)(10): Each employee engaged in roofing activities, with unprotected sides and edges 6 feet (1.8 m) or more above lower levels was not protected from falling by guardrail systems, safety net systems, personal fall arrest systems, or a combination of warning line system and guardrail system, warning line system and safety net system, or warning line system and personal fall arrest system, or warning line system and safety monitoring system:       On or about May 9, 2013, employees on the roof of the north side entrance were exposed to a fall of approximately 12 feet in height to the unpaved ground below, while installing Single-Ply TPO material.

29 CFR 1926.1053(b)(1): Portable ladders were used for access to an upper landing surface and the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface to which the ladder was used to gain access:       On or about May 09, 2013, employees were exposed to a fall of approximately 12 feet in height to the ground below while using a Louisville extension ladder, that had the side rails approximately 16 inches beyond the landing, to access a roof.
