29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system: (a) Jobsite: 232 Nemacolin Road, Carmichaels, PA 15320: On or before May 9, 2013, employees removing and/or installing roofing materials were working unprotected and exposed to fall hazards that ranged from approximately 8 feet 5 inches to 10 feet measured from the ground to the structures eave while on an 8/12 pitched roof.

29 CFR 1926.503(a)(1): The employer did not provide a training program for each employee potentially exposed to fall hazards to enable each employee to recognize the hazards of falling and the procedures to be followed in order to minimize these hazards: (a) Jobsite: 232 Nemacolin Road, Carmichaels, PA 15320: On or before May 9, 2013, the employer did not train their employees on the hazards associated with falling while removing and applying roofing materials on a steep sloped roof and what fall protection methods shall be used. Employees were anticipating that roof jacks and wooden plank would prevent a fall.
