On or about May 9, 2013 the employer provided a metal electrical enclosure in which was installed an electric receptacle for employee use in Operatory Room 1 where the electrical enclosure was not securely fastened in place.  b.    On or about May 9, 2013 the employer provided a metal electrical enclosure in which was installed an electric receptacle for employee use in Operatory Room 2 where the electrical enclosure was not securely fastened in place.  c.    On or about May 9, 2013 the employer provided a metal electrical enclosure in which was installed an electric receptacle for employee use in Operatory Room 3 where the electrical enclosure was not securely fastened in place.

On or about May 09, 2013, the employer had developed and implemented a written exposure control plan for employees with occupational exposure to blood or other potentially infectious materials but did not include in that plan the exposure determination required by 1910.1030(c)(2).
