29 CFR 1910.147(c)(4)(ii): The procedures shall clearly and specifically outline the scope, purpose, authorization, rules, and techniques to be utilized for the control of hazardous energy, and the means to enforce compliance including, but not limited to, the following: 929 Ridge Rd., Duson, L: On or about May 9, 2013, the employer's lockout tagout program did not specifically address the procedural steps for isolating or shutting down machines within the shop. Employee(s) were repairing or performing maintenance on a CNC machine which is electric and hydraulic controlled. Employee(s) were exposed to an amputation hazard.
